To facilitate your first visit, please bring the following with you: 

1. Your intake paperwork (New Patient Health History).  Please click below to download the forms and fill them out prior to your visit. 
2. Copies of any labwork done in the last 12 months.
3. A detailed diet diary for one week prior to your visit, if possible.  (Click below for a sample form.)
4. A detailed list of all the supplements and medications you take on a regular basis.
